Why do you have to wait 6 months after Accutane?

Accutane (Isotretinoin) is a well-established oral medication commonly prescribed for oily skin and severe acne patients. Patients with thick sebaceous skin may notice a thinning of their nasal skin with Accutane therapy as well.

• Therefore, patients need to discontinue Accutane for at least 6 months before any elective surgery involving the skin, including the nose.
• This precaution stems from several case series published in the 1980s that described delayed healing and keloid formation in patients undergoing surgery while taking Accutane.

While subsequent studies have shown that the actual risk for poor healing from surgery while taking Accutane is low, most surgeons would still advise waiting at least 6 months after stopping Accutane out of an abundance of caution, particularly for elective surgery.

Furthermore, Accutane can dry out mucus membranes and dry out the nose, which would make for a more difficult recovery after surgery. Along similar lines, we recommend that our patients wait at least 6 months AFTER rhinoplasty to allow the nose time to heal before resuming Accutane. Topical retinoid preparations, such as Retin-A, can be safely continued until 2 weeks before surgery and do not impair the healing process.

In fact, patients can consider starting a topical retinoid regimen 4-8 weeks before planned surgery to optimize the skin for better healing and less noticeable scars.

Dermatologists weigh in on why you shouldn’t sign up for Accutane, just to get a “free nose job.” Accutane, aka isotretinoin, is a powerful medication used to treat a particular type of severe acne known as recalcitrant nodular acne, according to the US National Library of Medicine’s MedlinePlus resource.

Patients may be prescribed Accutane when other treatments, like antibiotics, haven’t helped. Accutane is a retinoid, and it works by slowing the production of certain natural substances that can cause acne. Accutane “also shrinks the sebaceous—oil-producing—glands,” board-certified dermatologist Ife J.

• Rodney, MD, founding director of Eternal Dermatology Aesthetics, told Health,
• Accutane is an oral medication, and you take it by mouth twice a day with meals for up to five months at a time, says Medline Plus.
• The short answer: No.

What to expect after finishing Accutane?

Nearly all patients who complete a course of isotretinoin may experience an occasional breakout after the treatment. Hopefully, any breakouts are minor and go away quickly on their own. For any breakouts that you do have, you can use the clindamycin lotion that you have already been using.

Does liver go back to normal after Accutane?

The liver test abnormalities are typically asymptomatic and transient and can resolve even with continuing therapy. Clinically apparent liver injury due to isotretinoin is exceedingly rare.

  • Recommended face cleansers: Neutrogena ultra gentle foaming or creamy wash; CeraVe foaming or hydrating cleanser; Cetaphil daily facial or gentle skin cleanser.
  • Recommended face/body moisturizers: Cetaphil Cream, CeraVe cream, Neutrogena Hydroboost.
  • Recommended body moisturizers: Cerave Cream, Cetaphil Cream, Aveeno Eczema Therapy Cream.
• Joint/muscle aches: If you need to take over the counter pain reliever we recommend using Advil (ibuprofen) or Aleve (naproxen). Avoid taking Tylenol (acetaminophen).
• Nosebleeds: Prevent by applying Aquaphor or Vaseline to the inside of your nose sparingly, or by using Ayr saline nasal gel.
• Dry eyes (especially if you wear contacts): Use lubricating eye drops such as Systane or Refresh.
• Sun sensitivity: Minimize sun exposure. Wear sun protective clothing. Wear SPF 30 or higher sunscreen daily and if outside for an extended amount of time make sure to reapply every 2 hours!

• Stop all other acne medications during your Accutane course. (Including antibiotics, prescription creams, and over the counter acne washes). Continue oral contraceptives and spironolactone if applicable.
• Avoid vitamins with high doses of Vitamin A (ok if it contains less than 5,000IU of Vitamin A).
• AVOID WAXING unwanted hair. Tweezing, threading and shaving are safe to continue.
• Do not have any chemical peels, laser treatments, body piercings, tattoos, or elective surgical procedures while on Accutane or for 1 month following completion.
• Do not donate blood while on Accutane.
• Limit alcohol use while on Accutane.
• It’s ok if you run out of your medication before your next appointment. Accutane stays in your system for several days and missing a few days (or even weeks) will not impact the final outcome!

Bloodwork/Labs Our office will check labs, including your liver function tests and your triglycerides, before you start treatment and after two months of treatment. If everything is normal, you will not need any further bloodwork. If something is abnormal, there is a small chance of more bloodwork.

• Rare side effects:
• Concerning: severe nausea, vomiting, diarrhea, abdominal pain, unusually severe headaches especially if accompanied by vomiting or vision changes, mood changes, personality changes, or depression – contact your doctor immediately!
• Females only

For females the most serious side effect is severe birth defects if you become pregnant while on Accutane (isotretinoin). There is no long term risk of birth defects after you finish your course and it has no effect on your ability to become pregnant in the future.

2. Cetaphil, Cerave, Ego, Obagi Luxe, Hydrate & many more Retinol, AHAs, BHAs, Ascorbic acid – put them aside during your course of Roaccutane as these products can increase skin irritation.

You can recommence these actives one at a time two weeks after stopping your course of isotretinoin.

Is it normal for isotretinoin to not work after 3 months

You may start to see results from isotretinoin in as early as 4 weeks. But it can take several months to see its full effects. If you don’t respond well to isotretinoin after taking it for 4 to 5 months, your healthcare provider may recommend taking it for longer.
